Student Fan Bus Information - Parent/Guardian Fan Bus Permission Slip

Proctor/Hermantown High School Mirage Girls Hockey Team has made it to State!!

This form is to reserve a spot on the bus for grades 6-12. Payment ($15) for the bus will be collected before riding down to the state tournament. Also, you must purchase online tickets prior to arriving for the game as well. Click on the link below to request a spot on our fan bus.
